This guide has been produced by Antares Consulting with the collaboration of Vifor Pharma, the Spanish Society of Hospital Pharmacy (SEHF) and the Spanish Society of Family and Community Pharmacy (SEFAC).
It is part of the SimbioTIC project, which is based on the principle of cooperation and aims to encourage hospital and community pharmacists to work collaboratively towards better patient care, guaranteeing its continuity.
The document aims to provide guidelines so that those hospital pharmacy services that, together with the community pharmacies in their area, wish to initiate collaborative projects, implement them in an appropriate manner, can obtain measurable results and obtain recognition from patients.
Aware of the difficulties involved in cooperating when there is no mutual knowledge between the two groups, the guide provides guidelines to encourage cultural change, generating confidence based on results.
